How to apply
Duke of Edinburgh Application Form
Students will take part in activities they may already participate in and further develop their skills but will also be expected to take on some new and exciting challenges including a two-day 15 mile self-sufficient expedition.
Achieving this award will be a demonstration of independence, organisational skills and commitment!
Students will be required complete an application form and a provisional plan sheet which can be found via the links below.
The first part of the application will be a chance for students express their interest in the award and why they wish to take part.
The second part of the application will be a plan sheet where students have a chance create their programme. Their plan must contain the following:
- Their planned activity and the goal they are working towards
- A method to track their progress e.g. assessments, diary of events or progress tracker.
- A person that is proficient in the chosen activity to monitor and assess their progress. This will be their assessor.
